From the Patch: "TARRYTOWN, NY — With the news Tuesday from the state that people 60 years old and up, along with teachers, can now make an appointment to get the coronavirus vaccine, the villages of Tarrytown and Sleepy Hollow have extended the deadline to participate in a pop-up clinic.
The clinic, at which the Moderna vaccine will be administered, will be held from 8 a.m. to 6 p.m. Saturday, March 20 at the Sleepy Hollow Middle School gymnasium, 210 North Broadway in Sleepy Hollow.
The second dose will be scheduled for the same time frame Saturday, April 17."
If you are 60 or older and/or a teacher interested in receiving the vaccine at the Tarrytown/Sleepy Hollow pop-up clinic, fill out your information in this registration form.
A Tarrytown or Sleepy Hollow staff member will contact you to schedule an appointment via phone or email.
Capacity is limited, so register as soon as possible. The deadline to register is March 15.
Filling out the registration form does not guarantee you an appointment. You will only be contacted if being offered an appointment.
Organizers said you must be available for both the March 20 and April 17 appointments in order to register."
